Jason Collins, M.D.
Dr. Jason Collins received his Bachelor of Science degree from the University of Michigan in Cellular and Molecular Biology and his Doctor of Medicine degree from the University of Illinois in Chicago. He completed his Residency in Internal Medicine at University of Illinois Hospital and his Fellowship in Gastroenterology and Hepatology at Howard University in Washington D.C. He has a particular interest in Inflammatory Bowel Disease.
Dr. Collins is board certified in Gastroenterology/Hepatology. He is a member of the American Gastroenterology Association, American College of Gastroenterology and Colorado Medical Society.
Dr. Collins moved to Glenwood Springs with his family in 2010. His two children attend Montessori school in Carbondale. He takes time whenever he can to watch them play piano, golf, tennis or practice ballet; just a few of their many activities. Dr. Collins, in his free time, tries to take advantage of all that Colorado has to offer. Skiing, snowboarding, mountain and road biking are a few of his favorite past times. He is also an avid reader and loves to cook.
Colleen Farnum, P.A.-C
Colleen Farnum earned her Master’s in Physician Assistant Studies at the University of Saint Francis in Albuquerque, NM. She began her career as a surgical PA in California before moving to the Roaring Fork Valley. She has enjoyed specializing in Gastroenterology with Dr. Collins since 2013.
Outside of work, Colleen can be found on mountain bike trails in warm weather or fatbiking on snow-covered trails in winter. She also loves photography, running, and snowboarding. Colleen, her husband, and daughter, reside in Carbondale. She looks forward to raising her daughter in this wonderful valley.
Lisa Moore, RN/Office Manager
Lisa Moore received her Bachelors of Science degree in Nursing from Southern Connecticut State University and Yale New Haven Hospital. She enjoys providing nursing services for the community and has worked as a nurse in the Roaring Fork Valley for over 23 years. Her husband of 30 years is a regional manager for Black Hills Energy. Their daughter resides in Salt Lake City, working for a large investment banking firm as a financial analyst. Lisa has fun hiking, 4 wheeling, cooking and watching Hallmark movies.
Kaitlyn Melody, MA
Kaitlyn Melody attended IntelliTec College in Grand Junction, Colorado where she received her Associates in Occupation Studies for Medical Assistant in 2008. She has worked in Pediatrics and Gastroenterology and has worked with Dr. Collins for 3 years
Kaitlyn grew up in New Castle and graduated from Rangely High School in Rangely, Colorado. Kaitlyn takes joy in watching Netflix, fishing, camping, hunting, and 4-wheeling with her husband, Derek, and their black lab, Elli. Most weekends you can find Kaitlyn and Derek working on remodeling their home and hanging out with their families.
Tara Deemie, Medical Receptionist
Tara received a Bachelor’s Degree in Liberal Arts from the University of Missouri-Kansas City. She moved to the Roaring Fork Valley in 2009 and has worked with Dr. Collins for 4 years. Her hobbies include hiking, mountain biking, camping and rafting. She loves cooking, reading, wine, dancing and enjoying all that Colorado has to offer.
Lyndsay Fangman, MA
Lyndsay graduated from the University of Montana with a Bachelor’s Degree in Health and Human Performance. She has lived in the Roaring Fork Valley for nine years and has a passion for serving this community. Outside of Roaring Fork Gastroenterology, Lyndsay enjoys mountain biking, hiking, four-wheeling, and assistant coaching the Glenwood Springs High School Cheer Squad.
Crystal Osborn, Medical Receptionist
Crystal Osborn comes to Roaring Fork Gastroenterology with 5 years of Medical Reception experience. Crystal is married and has 3 grown children. Her oldest son is a software engineer in Washington. Her second son is self employed locally as a carpenter. Her daughter is a retail assistant manager in Glenwood Springs. Crystal enjoys spending time with her family, hiking with her dogs, cooking, shopping and church activities.
